Age Requirement for Registration

◘Pre-Kindergarten: Must be four (4) on or before September 1st.

◘Kindergarten: Must be five (5) on or before September 1st.

◘1st Grade: Must be five (6) on or before September 1st.

Housing Lease Agreement or letter from housing verifying family is on a waiting list

•If military sponsor is not the biological parent of enrolling student, additional information may be required such as a marriage license, spouse's ID card, and custody/court documents.

•Sponsor is required to sign the form 600. Spouse may sign with a Power of Attorney attached.

Withdraw Notification

If students are withdrawing from Elliott Elementary, parents are to notify the school registrar five days in advance by filling out a withdrawal form.

Certificate of Immunization

Students who enroll in DoDEA schools MUST meet specific immunization requirements. These requirements, displayed below, represent the minimum requirement and do not necessarily reflect the optimal immunization status for a student. The DoDEA Immunization Requirements Form 2942 is provided to parents for informational purposes. This form may be used by medical officials but it is not required.  However, some type of medical proof of immunization must be completed by medical authority and provided to school officials at the time of initial registration.

Diphtheria, Tetanus, Pertussis (DTP)

Four (4) doses with at least one dose after the 4 th birthday

Hepatitis A

Two (2) doses at least 6 months apart

Hepatitis B

Three (3) doses

Haemophilus influenzae type b (Hib)

Two (2) to four (4)  doses before age 5

Polio

Three (3) doses with at least one dose after the 4th birthday

Menigococcal

One dose after age 11 or 12

Measles, Mumps, Rubella (MMR)

Two (2) doses.

Varicella

One (1) dose or documented history of chickenpox
