Registration Information

Forms required to determine eligibility and register for a DDESS school.

Certificate of Immunization

Students who enroll in DoDEA schools MUST meet specific immunization requirements. These requirements, displayed below, represent the minimum requirement and do not necessarily reflect the optimal immunization status for a student. The DoDEA Immunization Requirements Form 2942 is provided to parents for informational purposes. This form may be used by medical officials but it is not required.  However, some type of medical proof of immunization must be completed by medical authority and provided to school officials at the time of initial registration.

Diphtheria, Tetanus, Pertussis (DTP)
Four (4) doses with at least one dose after the 4 th birthday
Hepatitis A
Two (2) doses at least 6 months apart
Hepatitis B
Three (3) doses
Haemophilus influenzae type b (Hib)
Two (2) to four (4)  doses before age 5
Polio
Three (3) doses with at least one dose after the 4th birthday
Menigococcal
One dose after age 11 or 12
Measles, Mumps, Rubella (MMR)
Two (2) doses.
Varicella
One (1) dose or documented history of chickenpox

How to obtain above requirements from Naval Hospital Beaufort

Call the routine appointment/central appointments line at (843) 228-5175 and ask to schedule an Immunization Clinic appointment.

The Immunization Clinic is located on the 2nd floor of the hospital.

Immunization and record reviews are available by appointment only:

  • Monday, Tuesday, Wednesday, and Friday: 0800 - 1200 and 1300 - 1600. 
  • Thursdays:  0800 - 1200 only.
  • The Immunization Clinic is closed on holidays.

Please take your child’s record of immunizations (shot card) with you to all immunization appointments. Please call the Elliott School Nurse at (843) 846-1265 if you have questions.

Withdraw Notification

If students are withdrawing from Elliott Elementary, parents are to notify the school registrar five days in advance by filling out a withdrawal form.
