- The following supplemental registration forms are also required. NOTE: complete and print a separate set of forms for each student in your family.
- These forms can be filled out online and printed.
- These forms are not fillable. Please print out and complete by hand.
- (optional) To participate in after school athletic programs, print and complete a Virginia Athletic Participation/Parental Consent/Physical Examination Form. It is not necessary to complete this form for regular P.E. classes.

It will facilitate your child's transition if you handcarry copies of records, including report cards, standardized testing, special education records (if any), and any other pertinent information to the Dahlgren office when you register. Do not pack these records with general household goods, but keep them somewhere accessible during the move.
Enrollment Information
All active duty military dependents, as well as DoD civilian dependents, in pre-K through eighth grade who reside in housing on the Dahlgren base are eligible and welcome to enroll in Dahlgren School (see Lincoln Military Housing for more information on base living).
DoDEA Kindergarten Registration Information
DoDEA, as a member of the Interstate Compact on Educational Opportunity for Military Children, is committed to easing the challenges of military families moving between duty stations. For families transitioning to a new duty station and enrolling in DoDEA schools for the first time, there is now a provision, allowing a 30-calendar day grace period for parents/ students to meet immunization requirements for school enrollment. The intent of this provision is to avoid any interruption in the transferring student's education. Compliance with immunization requirements greatly reduces the risk of public health issues. School nurses and local military medical treatment facilities stand ready to assist parents in meeting these requirements promptly.
