DoDEA Kindergarten Registration Information
A child must be four years old by September 1 to attend pre-kindergarten or pre-school five years old by September 1 to attend kindergarten and six years old by September 1 to attend first grade.
Child’s Birth Certificate: A legible copy of original State birth certificate. For children born overseas, an original Form FS-240, “Report of Birth Abroad of a Citizen of the United States of America” or an original Form DS-1350, “Certification of Birth Abroad” will be accepted.
Health History Form (DS Form 12.1, revised May 2002) - to be completed by the parent/guardian at the time of registration.
Up-To-Date Immunization Record (a legible copy).
Varicella vaccine is required for all children age 1 and older who have not had the Chickenpox disease. Hepatitis A is required for all children at 1 year of age. Haemophilus influenzae type B is required for children up to 5 years of age. Four-year booster immunizations are recommended for all children entering pre-school.

The sponsor’s CURRENT military orders.
A copy of the sponsor’s current military orders may be obtained by contacting the Military Personnel Division (MPD), located in building 622. Telephone:938-8489.
Documentation of the sponsor's housing assignment in permanent quarters with CURRENT housing lease or a 90-180 Day Housing Waiver/Eligibility Impact Letter to WPES School registrar for your child to be eligible to attend SY 11-12.
