Immunizations/Physicals Exams:

nurse tools

All students must present an official immunization record that is current  according to DoDEA Immunization Requirements prior to enrollment in school. Parents should have the child’s
immunization record reviewed by the medical clinic or the nurse at the school to
determine if any immunizations will be needed to bring the record up-to-date.
If shots are needed, these must be received and documentation submitted to the school
prior to the child’s first day of attendance.

Health Screenings:

eye chart
Typically, health screening is conducted early in the school year so that problems can be identified and corrected quickly. This screening typically includes: vision, hearing and height/ weight measurements.
Referrals are made for medical follow-up as needed.
Students are also screened throughout the
school year at the request of teachers and
parents. It is recommended that if a parent
has concerns about a child’s hearing or
vision, that these be addressed with the medical clinic before the child begins
school.

Health Concerns:

Epipen
Upon enrollment and at the beginning of each school year parents are asked to complete a Health Information and Medical Permission Form.  Please make sure that this form is completed and returned so that your child's health concerns can be noted and addressed.  This form also gives permission for your child to receive first aid, screenings and comfort measures at school.

Medications at School:

medicine
In order for any medication, prescription or Over the Counter (OTC) drug, to be administered at school, the following must occur:
  • A Physician's Medication Form must be brought to school by the parent and logged in. It is against school policy for students to transport their own medications.
  • Medications must be in a pharmacy-labeled container.  The label should contain the child's name, medicaiton name and dosage adn time to be administered.  Womack pharmacy will label OTC meds such as Motrin, Tylenol and Sudafed when ordered by the doctor.

Please do not send sick children to school.  Not only will the child have difficulty learning, he or she will put other students and teachers at risk for illness. Typically a child should remain at home if:
  • the child has a temperature of 100 or above.  Once the temperature is below 100 without meds, the child may return.
  • vomiting has occurred twice within the last 24 hours.
  • diagnosed with strep throat.  Once antibiotics have been given for 24 hours and a child is fever free, he/she may return.
  • diagnosed with ringworm until antifungal treatment is started.
  • diagnosed with chickenpox until all lesions are crusted and the child is fever free (usually 7 days.)
  • eyelashes are heavily crusted upon waking.

Emergencies:

ambulance
Please keep your phone numbers and emergency contact numbers current and accurate at school.  It is best to give changes in these contact numbers to the school secretary or the nurse so that the computer can be updated.  In cases of illness or injury it is imperative that we be able to locate someone to pick up the child.
