2009-2010 NEW STUDENT REGISTRATION

The list of documents below is required for registering your child.  Please attach all documents to your registration packet when registering your child.

       1.  DoDEA Form 600 (in the registration packet)

        2.  Emergency Contact name, address and phone numbers.

       3.  Power of Attorney if the sponsor is not present to enroll the student.

       4.  Birth Certificate – certified copy. (Make copy for file and return original.)
     If the sponsor is the biological parent and not listed on the birth certificate, one of the following documents are required:

                                        Dependent is listed on orders
                                        Student ID – reflects sponsor’s name
                                        Student is listed as dependent on approved DEERS form
                                               
       5.  If the sponsor is not the biological parent of the student, the following document is required:           
                                                   Marriage Certificate

       6.  Copy of the Sponsor’s Military Orders assigning him/her to Fort Campbell.

       7.  Housing Authorization
School housing letter with child’s name listed.  Your family must be living on post housing or have a letter from housing stating that the family will be in housing within 180 school days.  *Failure to accept on-base housing will require the children to be withdrawn from on-base schools.

       8.  Physical Examination – Physical should not be over 1 year old.  (Physicals are due  no more than 30 days from enrollment date.)

       9.  Certificate showing updated and required immunizations. 
                  DoDEA/Fort Campbell Certificate of Immunization.  Student will not be able to attend school without this certificate.  These may be obtained from your child’s  health clinic.  (Red, White, Blue, or Young Eagle)

      10.  Signature Forms Returned:  (Registration Packet must be picked up in the office.)

      11.  Records Request Release Form is required.

      12.  Transcript required.  Freshman – Report Card showing promotion to the 9th grade.
