2008-2009 NEW STUDENT REGISTRATION CHECKLIST

1. Age requirement

Pre-school (4) years old on or before October 1st.

Kindergarten (5) years old on or before October 1st.

First Grade (6) years old on or before October 1st.

2. Birth Certificate

3. Health History

4. Certificate showing updated and required immunizations We accept Kentucky and DoDEA Immunization Certificates. The Immunization Clinic is open for walk-ins Monday and Wednesday 7:30 -11:00 and 12:30 - 4:00; Friday 12:30 - 3:00. Closed Tuesday and Thursday.

*Additional requirements:

Students grades 6th through 12th are required to complete Hepatitis B inoculation series.

All students 11 years old or older are required to complete the Meningococcal/Conjugate Vaccine, Tetanus and Diptheria Toxoids and Acellular Pertussis Vaccine (TDAP Adolescent Preparation.)

6. Housing Authorization

Your family must be living in post housing or have a letter from housing stating that the family will be in housing within 180 school days. * Failure to accept on-post housing may require the children to be withdrawn from on-post schools.

7. Copy of the Sponsor's Orders assigning them to Fort Campbell or copy of Sponsor's active duty ID.

8. Name and address of school last attended, if applicable

9. Emergency Contact name, address and phone numbers

10. Power of Attorney if the sponsor is not present to enroll the student

11. If the sponsor is not the biological parent of the student the following documents are required :

Marriage Certificate

Custody Papers

All students entering the 6^th grade or higher will require evidence of completion of the Hepatitis B inoculation series.
