Immunization Requirements

Immunization against communicable diseases is required of all students at Fort Campbell.  Protection is required against diphtheria, pertussis, tetanus, measles (rubella), and poliomyelitis.  Kentucky State required doses are as follows:
            Age 4 or over:
            Diphtheria, Pertussis, Tet (DPT)…………     5 doses*
            Oral Polio……………………………………………………4 doses
            DPT. TD…………………………………………………… .4 doses
(if the fourth dose was given after the 4th birthday, if not, 5 doses are required)
            MMR………………………………………………………...  1-2 doses
            (Second dose at age 4))
            Hib (Haemophilus influenza type b conjugate vaccine    4 doses
            Hepatitis B series of 3 doses, 1st dose in series required for initial enrollment
            Varicella (Beginning August 1, 2001)
            (Or doctor’s statement that the child has had chicken pox)
            Meningococcal Conjugate Vaccine
            Tetanus and diphtheria Toxoids and Acellular Pertussis Vaccine (TDAP-adolescent
            preparation)
All students must have a second MMR (Measles/Mumps/Rubella) within one year prior to entering the 6th grade unless two (2) doses of MMR were previously received.

Immunization certificates may be obtained at the Immunization Clinic, Third Floor-C Building, Blanchfield Army Community Hospital (BACH).  A Kentucky Certificate is required for enrollment.  Walk-in Clinic Hours:  Monday and Wednesday  -7:30-11:00 & 12:30-4:00
Friday  - 12:30-3:00
On February 1, 2001, the interim Joint Committee on Health and Welfare approved an amendment to the school immunization requirements for varicella, and hepatitis B.  The implementation schedule is described below.

VARICELLA:  Beginning August 1, 2001, all children at least (19) months of age and less than seven (7) years of age who attend day care centers, certified family child care homes, other licensed facilities which care for children, preschool programs, and public or private schools shall have one dose of varicella vaccine, unless a parent, guardian, or physician states that the child has had chickenpox disease.

HEPATITIS B:  Effective August 1, 2001 and through the 2008-2009 school year, all public or private primary schools shall require for sixth grade entry, two (2) doses of hepatitis B vaccine separated by no less than four (4) weeks, and a third dose four (4) to six (6) months after the second dose.  If an accelerated schedule is needed, the minimum interval between the first two (2) doses shall be four (4) weeks, and the minimum interval between the second and third doses shall be (18) weeks.  The first and third doses shall be separated by at least four (4) months.  A current immunization certificate shall be on file at time of initial enrollment.

Medical and religious exemptions will still be accepted.
