Required Documents for DDESS Eligibility and
Registration
The following documents
are required to determine eligibility and register a child
in the DDESS schools:
- A certified birth certificate from
the Department of Vital Statistics of the state of birth.
The certificate must include the parents’ names and
cannot be a pocket card or hospital/congratulatory certificate.
A copy of the certificate will be retained.
- Instructions
for obtaining birth certificates from all 50 states may
be found at the National Center for Health Statistics
web site: http://www.cdc.gov/nchs/howto/w2w/w2welcom.htm.
- If the sponsor is not listed on the birth certificate
as a biological parent, the child's dependency and custody
determination must be documented before enrollment is
granted. Please contact the school or district registrar
for a list of required documents.
Note: The governing statute and instruction require that children who are
not the biological children of the registering sponsor
be their legal dependents. In order to establish this
relationship, there must be court-ordered custody/guardianship
assigned to the sponsor and/or his/her spouse. A power
of attorney or notarized statement from the child’s biological
parent does NOT meet the requirements of the statute and
instruction.
- The child's social security card. This document
will be copied and retained.
Note: The form for applying
for new or replacement social security cards may be found
on the Social Security Administration's website: http://www.ssa.gov/online/ss-5fs.pdf.
- The sponsor’s current military orders. Orders dated
36 months or more prior to the beginning of next school year
will require an amendment, updated orders, or the school/district
registrar can provide a form to be completed by the sponsor’s
personnel officer.
- In the Continental United States (CONUS)
ONLY: documentation of the sponsor's
housing assignment in permanent quarters, either by:
- a housing letter
- Housing Assignment Voucher, or
- the military housing lease agreement from the privatized housing partner (where applicable).
The document must list the child as a dependent with authorization to permanently live in housing. No child will be permitted to start school without this documentation or:
- a letter from the Housing Office certifying that housing will be available within 90 school days,
- the sponsor is on the active waiting housing list and intends to accept housing when offered.
AND
- verification that the child will be authorized to reside permanently in
the housing when assigned.
Enrollment under a 90-School Day letter is provisional; dependents of sponsors who
remove their names or decline housing will be disenrolled.
Additional eligibility documentation is required in the
territories, possessions and commonwealths such as Puerto
Rico and Guam.
DoDEA Form 600 is used to collect required registration data
which is then entered into the automated Student Information
System for use by DoDEA personnel in providing educational
and management programs.
Local districts and schools may require
additional documents be
submitted during the registration process. Most DDESS
schools list the required documents on their websites.
