Information for PCSing Families

Fort Jackson Elementary Schools Registration Information

Required Documents to Enroll

  • Lease or Memorandum from Housing Office verifying residence or residency assignment within 90 days.
  • Student's original birth certificate or certified copy.
  • Student's original social security card.
  • Form 1172 (DEERS Enrollment Form) is required if the student's legal last name differs from the sponsor. (This form can be obtained from the Welcome Center).
  • South Carolina Certificate of Immunization (DHEC-1148). The following immunizations are required:
    • DTP: Four (4) doses. At least one dose must be administered after the 4th birthday.
    • Hepatitis B: Three (3) doses
    • MMR: Two (2) doses
    • Polio: Three (3) doses. At least one dose must be administered after the 4th birthday.
    • Varicella: One (1) dose.
  • Fort Jackson Schools' Registration Forms.
  • Names, addresses, & telephone numbers for sponsor, spouse, emergency contacts, command POC's, and next of kin.

Withdrawal Notification

If students are withdrawing from Fort Jackson Schools, parents are to notify the school secretary at 782-1772 five (5) days in advance.

How to Obtain Medical Forms at Moncrief Army Community Hospital

  • Immunization and record reviews are available Monday-Friday from 7:30 AM to 3:30 PM.
  • The immunization clinic is located on the 6th floor.
  • The clinic's telephone number is 803-751-2434.
  • Vision and Hearing screenings are available by appointment Monday - Friday. To schedule an appointment, call our school's nurse at 787-8288 x216.
  • Dental screenings are not needed, nor are they available on post.
