FKCS Medical Requirements for Attending School

Immunization Certificates:

All students enrolled in the Fort Knox community Schools must have a Kentucky Immunization Certificate to include a recent TB skin test. The TB skin test must have been done within the last 12 months stateside, and the last 6 months overseas. All students who attend Fort Knox Community Schools are eligible to receive their immunizations at the Ireland Army Community Hospital Immunization Clinic. The Immunization Clinic is open from 7:30-3:00 and NO appointment is needed. However, TB tests are not given on Thursdays. ONLY those students enrolling after school begins are given two weeks to obtain the certificate (Discovery students must have theirs PRIOR to starting school). Failure to comply will result in exclusion from attending school until this requirement is met.

The immunization certificates are due on the first day of school.

School Physicals:

Only those students listed below are required to turn in a school physical.

  1. DISCOVERY: Four-year-old children who are being enrolled in the Discovery Program must have a Kentucky Immunization Certificate, a school physical and a vision exam (not a screening), turned in to the school prior to attending. For clerical purposes, the paperwork for discovery students is not filed and the child does not go on the school roster until all completed paperwork is received. The school physical must have a date within 12 months of entering school for the first time. The physicals are done in the sponsor's Primary Care Clinic, Call 1-800-941-4501 for an appointment. Physicals by other providers are acceptable if they are dated within 12 months of entering school for the first time. After the initial physical is done for the Discovery Program, no further physical exams are required until the sixth grade.
  2. KINDERGARTEN AND FIRST GRADE : Students enrolling in kindergarten and first grade and who do not have a physical exam report in their school records, must have a school physical done upon inital enrollment to school. Kindergarten and first grade students must have their physicals or an appointment slip presented to the nurse no later than 30 days after they enter school. Failure to comply will result in exclusion from attending school until completed. Physicals must be dated within one year prior to entering school for the first time. Kindergarten and first grade physicals are done at the sponsor's Clinic at Ireland Army community Hospital. Call TRICARE at 1-800-941-4501 or the appointment line at 1-800-493-9602 for an appointment. Other care providers may do physicals as long as they are dated within one year prior to initial enrollment to school.

Information on Sports Physicals:

If your child plans to participate in school sports, he/she must have a sports physical. All sports physicals must be done in the sponsor's Primary Clinic. Call 1-800-941-4501 for an appointment. You must pick up the form for this physical at Fort Knox High School or Scott Middle School prior to the appointment. Sports physicals must be done after July 1 of the current school year.
