Medical Requirements for Attending School
Discovery - Third Grade
IMMUNIZATION CERTIFICATES
All students enrolled in the Fort Knox Community Schools must have a Kentucky Immunization Certificate. The Immunization Certificate is issued by the Immunization Clinic at Ireland Army Community Hospital. All students who attend Fort Knox Community Schools are able to receive their immunizations at the Immunization Clinic. Hours at the Immunization Clinic are:
Monday- Friday 0730-1130 and 1300-1530 No Appointment Necessary
The immunization certificates are due on the first day of school. Any student enrolling after the first day of school has two weeks to obtain a certificate. The school nurse can write the certificate if all the shots are up to date and no other immunizations are required. Any student who fails to meet the above guidelines will be denied attendance to school until the certificate is received.
SCHOOL PHYSICALS/VISION EXAMS
DISCOVERY PROGRAM
Four-year-old children who are enrolled in the Discovery Program must have a Kentucky Immunization Certificate, a school physical and a vision exam (not a screening), turned in to the school prior to attending. For clerical purposes, the paperwork for discovery students is not filed, and the child does not go on the school rolls, until it is completed.
PHYSICALS: The school physical must have a date within 12 months of entering school for the first time. The physicals are done in the sponsor’s Primary Care Clinic, Call 1-800-493-9602 for an appointment. Physicals by other providers are acceptable if they are dated within 12 months of entering school for the first time. After the initial physical is done for the Discovery Program, no further physical exams are required until the sixth grade.
VISION EXAMINATIONS:
People with TRICARE PRIME can receive this service at no charge from the following providers:
1)
20/20 Eyecare in Elizabethtown. It is located in the Starlight Shopping Center behind Tumbleweed Restaurant across from the Towne Mall in Elizabethtown. Call 270-765-2020 for an appointment. You must have an appointment scheduled and give the school the date prior to your child attending school.
2)
Dr. John Roberts in Radcliff. His office is located next door to Hardin Eye Wear on Lincoln Trail Blvd. in Radcliff. His phone number is 270-351-8661.
3) The Optometry Clinic at Ireland Army Community Hospital. Call 1-800-493-9602 for an appointment. Appointments are space-available only for family members.
4) Wal-Mart Vision Center in Radcliff. Call 352-2707.
KINDERGARTEN AND FIRST GRADE STUDENTS
PHYSICALS: Students enrolling kindergarten and first grade and who do not have a physical exam report in their school records must have a school physical done upon initial enrollment to school. Parents are given 30 days to get the school physical. Physicals must be dated within one year prior to entering school for the first time. Kindergarten and first grade physicals are done at the sponsor's Primary Care Clinic at Ireland Army Community Hospital. Call 1-800-941-4501 for an appointment. Other care providers may do physicals as long as they are dated within one year prior to initial enrollment to school. If a school physical exam report or an appointment slip is not presented to the school within 30 days of entering school, the child will be denied attendance until an appointment slip is presented to the school. After the initial physical is done in Kindergarten or First Grade, no further physicals are required by the school until the sixth grade.
VISION EXAMINATIONS : Kindergarten students enrolling in the Fort Knox Community Schools must also have a comprehensive vision exam done by a doctor if they have not already had one done.
