Fort Knox Community Schools Medication Policy
The Fort Knox Community Schools require that all students who need medication during school hours adhere to the following:
- The parent or legal guardian must sign the "Permission To Give Medication At School" consent form.
- Medication must be brought to school by the parent or guardian in the original prescription bottle, properly labeled by a registered pharmacist to the school nurse. Children will not transport medication to or from school.
- The nurse will remove the amount of medication needed for school to complete the treatment and send the original home.
- For long term medication (longer than 4 weeks), the nurse should be provided with a separate prescription bottle which is labeled "For School Use" or has the initials ESBP (extra school bottle please) written on the label. In the event that the nurse is not available, the school principal or his/her designee will give the medication.
- Bacitracin Ointment is used for cuts and scratches when the nurse feels it is indicated. Calamine lotion is kept on hand for use on poison ivy/oak.
- No over the counter or nonprescription drugs will be given by the nurse. The nurse does not keep Tylenol or Tums for student use.
Medical Requirements for Attending School
Immunization Certificates
All students enrolled in the Fort Knox community Schools must have a Kentucky Immunization Certificate [125KB] to include a recent TB skin test. The TB skin test must have been done within the last 12 months stateside, and the last 6 months overseas. The Immunization Certificate is issued by the Immunization Clinic at Ireland Army Community Hospital. All students who attend Fort Knox community Schools are eligible to receive their immunizations at the Immunization Clinic. TB tests are not given on Thursdays. Hours at the Immunization Clinic are Monday- Friday 0730-1530
No Appointment Necessary
The immunization certificates are due on the first day of school. Any student enrolling after the first day of school has two weeks to obtain a certificate. Any student who fails to meet the above guidelines will be denied attendance to school until the certificate is received.
School Physicals
Only those students listed below are required to turn in a school physical.
Discovery Program: Those four year old children who qualify for this program must have a school physical and Kentucky Immunization Certificate turned in to the school prior to attending. These physicals are done in the sponsor’s Primary Care Clinic, Call 1-800-941-4501 for an appointment. After the initial physical is done for the Discovery Program, no further physical exams are required until the sixth grade. Kindergarten and First Grade Students: Students enrolling in kindergarten and first grade and who do not have a physical exam report in their school records, must have a school physical done as soon as it can be arranged. Kindergarten and first grade physicals are done at the sponsor’s Clinic at Ireland Army community Hospital. Call 1- 800-941-4501 for an appointment. If a school physical exam report or an appointment slip is not presented to the school within 30 days of entering school, the child will be excluded from school until an appointment slip is presented to the school.
Sixth Grade Students: All sixth grade students are required to have a school physical. Sixth grade physicals are done at the sponsor’s Primary Clinic at Ireland Army Community Hospital. Please call 1-800-941-4501 for appointments with the Primary Clinics. Students must have the physical exam report the first day of school to attend or have scheduled an appointment if enrolling after school starts. Previous physicals are acceptable if they were done within one year of entering the sixth grade.
Sports Physicals
If your child plans to participate in school sports, he/she must have a sports physical [156KB]. All sports physicals must be done in the sponsor’s Primary Clinic. Call 1-800-941-4501 for an appointment. You must pick up the form for this physical at Fort Knox High School or Scott Middle School prior to the appointment. Sports physicals must be done after July 1 of the current school year
