Important Information Regarding Immunization and Health Assessment Requirements for this SY

Immunizations, per DDESS guidelines, must be up-to-date for all CLDS students by Friday, September 19, 2008. Students who have not received required immunizations will be suspended from school.

A current Health Assessment must be completed for all Pre-K students NLT Friday, October 3rd. The health assessment must be turned into the child's school. For additional information, please contact your School Nurse.
