medicine bottleNew Medication Administration Permission Form for SY 2007-08

A new DoDEA Medication Administration Permission Form will be required to accompany any medication that students are expected to take while at school. Physicians will complete the top portion and parents complete the bottom. This form has been distributed to our Naval Hospital and also local pediatricians in Onslow County.

 

Important News Regarding Immunizations:

 

By September 19th all immunizations MUST be in compliance with the DoDea regulations.

Your student must have:

  • At least #1 in the HEP A series
  • All other immunizations must be up to date.

 

Pre School students must have:

  • At least 4 DTP’s
  • 3 IVP’s
  • 3 HEP B
  • 1 HIB
  • 1 MMR
  • 1 Varacella

ALL other students MUST have 5 DPT’s (unless #4 given late), 4 IVP’s, 3 HEP B, HIB, 2MMR’s and at least 1 Varacella.

Reminder:

 

NEW IMMUNIZATION REQUIREMENT BEGINNING SY 07-08

EACH student is to have the Hepatitis A Series, as required by the DoDEA Immunization Policy. Parents if your new to JPS your student must have the first immunization in the series with in TEN DAYS of your child's first day of school.

The second immunization is due 6 months after the first and no later then 18 months after. Parents if your student had his/her 1st immunization and it is time for the second please go to the clinic and have this done. If you are not sure check with Mrs. Russell, RN JPS School Nurse. It is important that your student get the second immunization when it is time so he/she will be ready for school in the fall. Please make sure that the school nurse gets a copy for your students health record.In the Fall of SY 08-09 children will have 10 days from the first day of school to be in compliance or be suspended per the DoDEA Immunization Policy.
