Enrollment Requirements,
Registration Forms**, and Supply List

All students from pre-k through eighth grade residing in military housing on the Dahlgren base are eligible and welcome to enroll in Dahlgren School (see Lincoln Military Housing for more information on base living). Please note the Entrance age eligibility requirement which will affect Kindergarten beginning in School Year 2009-2010.

New students will need to complete registration forms** (see below) and provide an original birth certificate, Social Security card, immunization record, and current (within one year of the first day of school) Virginia Physical Examination certification. Virginia School Law states that children are of school age if they have reached their fifth birthday on or before September 30 in the year that they will be enrolled.

IMPORTANT NOTE: It will help Recordkeeping immensely if sponsors obtain copies of their children’s records (report cards, standardized testing, special education records (if any) and any other pertinent information such as portfolios) and HANDCARRY THESE RECORDS TO DAHLGREN WHEN REGISTERING THEIR CHILD. Do not pack these records with general household goods, but keep them somewhere accessible during the move.

New students must also have an updated Virginia physical examination dated within one year of the first day of school. This includes pre kindergarten students who must have another physical before entering kindergarten. Other returning students are not required to have a new physical; however, immunizations updates are required.

The 1991 Virginia General Assembly passed legislation requiring that all children must receive a second dose of measles vaccine before entering kindergarten or first grade. The 1992 legislation required that all children who have not yet received a second dose of measles (rubella) vaccine must receive a second dose before entering the sixth grade.
