The information below includes documentation required by our schools. Each school may have additional forms they use so please see the links at the bottom of this page.

Registration Information

Date of Birth Verification:
One of the following.
Age Requirements
- 4 years old by September 1 to enroll in the Pre Kindergarten program
- 5 years old by September 1 to enroll in Kindergarten
- 6 years old by September 1 to enroll in First Grade
DoDEA Policy - Minimum Age Requirement for Pre-Kindergarten, the Sure Start Program, Kindergarten, and First Grade Dependents
Verification of Student is an eligible Dependent:
One of the following.
- Sponsor is listed as biological parent on birth certificate
- Child is listed on sponsor’s orders
- Sponsor is named on child’s military ID
- Approved DEERS registration form
- If sponsor is a step-parent, and one of the above cannot be provided, the following is required:
- Birth certificate documenting sponsor’s spouse is biological parent
- Custody documents indicating spouse has full custody of child
- Marriage certificate of sponsor and child’s biological parent
- If neither the sponsor nor spouse are biological parents, court ordered custody or guardianship documents are required (power of attorney or notarized agreements between biological parents and sponsor are NOT accepted).
- Note: If court order names only the spouse as guardian or custodian of the child, the sponsor will be required to complete and sign an in loco parentis affidavit (available from the school registrar).
Verification of Active Duty Status (military only):
One of the following.
- Current orders (less than 3 years old at most locations)
- Sponsor's active duty identification card
- Letter from sponsor's command certifying active duty status
- Current deployment orders
Verification of Full-Time DoD Employment (civilians only):
One of the following.
- Current SF 50
- Letter from servicing HR Office
Verification of Housing:
One of the following.
- Housing assignment letter, voucher, or RCI lease agreement
- Letter from housing office that housing will be available and offered within 90 school days.
DoDEA Immunization Requirements:
One of the following.
DoDEA, in alignment with the Interstate Compact on Educational Opportunity for Military Children, is committed to easing the challenges of military families moving between duty stations. For families transitioning to a new duty station and enrolling in DoDEA schools for the first time, there is now a provision, allowing a 30-calendar day grace period for parents/students to meet immunization requirements after school enrollment. The intent of this provision is to avoid any interruption in the transferring student’s education. The health and the welfare of all students is a top priority for DoDEA. The Department of Defense sets immunization requirements for DoDEA schools based on recommendations from the Center of Disease Control. Compliance with immunization requirements greatly reduces the risk of public health issues. School nurses and local military medical treatment facilities stand ready to assist parents in meeting these requirements promptly
