Dear Sponsor,
Welcome to the West Point Middle School. We strive to provide exemplary
educational programs that inspire and prepare each student for SUCCESS
in a global environment. When you visit us for registration you will receive
a registration packet that will have to be completed, and returned to
our school.
Included in this packet is a Registration Form (DoDEA Form 600). Please
ensure that your child's social security number is entered correctly and
that the form is signed by the active duty sponsor. If the sponsor is
deployed, it may be signed by his or her spouse with a Power of Attorney
(a copy will need to be submitted with the documents). If this is not
possible, please ask the school registrar for further assistance.
In order to register your child, you will need to provide the following
information. Without this information, your child will not be permitted
to attend West Point Middle School.
• A certified birth certificate from the Department of Vital Statistics
of the state of birth. The certificate must include the parent's names
and cannot be a pocket card or hospital/congratulatory certificate. A
copy of the certificate will be retained.
• If the sponsor is not listed on the birth certificate as a biological
parent, the child's dependency and custody determination must be documented
before enrollment is approved. Please contact the Registrar for a list
of required documents.
• The child's social security card. This document will be copied
and retained.
• Individual Shot Records
• IMMUNIZATION REQUIREMENTS FOR DOD SCHOOL ENTRY -Students who
enroll in DODEA schools MUST meet immunization requirements unless a medical
or religious waiver is completed by child’s physician. The immunization
requirements follow the guidelines from the Center for Disease Control
(CDC) and are based upon recommendations from the Advisory Committee on
Immunization Practices (ACIP).
Tdap, DPT, DTaP, DT, or TD - a minimum of four doses. A child must have
at least one dose of DPT after the 4th birthday. The usual schedule is
a primary series of four doses at 2m, 4m, 6m, and 15-18m of age. If the
fourth dose of DTaP, DT, or DPT is administered before the 4th birthday,
a booster (fifth) dose is recommended.
***A single Tdap booster dose is recommended for children 11-12 years
old, if 5 years elapsed since the last dose; then every 10 years with
the Td.
Hepatitis A. The two dose series administered at least 6 months apart.
Hepatitis B. The three dose series is given on standard schedule of 0,
1 and 6 months.
Meningococcal Vaccine. Meningococcal conjugate vaccine (MCV4) should be
given to all children at age 11-12 visit as well as to unvaccinated adolescents
at high school entry
(15 years old). Vaccination is also recommended for children and adolescents
> 2 years with terminal complement deficiencies or certain high risk
groups.
Polio (OPV or IPV) - three doses. At least one dose must be administered
AFTER 4th birthday. Usual schedule is primary series of four doses at
2m, 4m, 6-18m, and 4-6 years of age. All doses should be separated by
at least four weeks. If dose three is given after 4th birthday, dose four
is not needed.
Hib – two to four doses. Primary immunization occurs at 2m, 4m,
6m, and 12-15m (booster dose); catch up schedule available; Hib is not
routinely given to children 5 years and older.
Measles, Mumps, Rubella - (MMR) - Two doses of MMR. The first dose administered
at 12-15 months of age. The second dose is recommended routinely at age
4-6 years, but may be administered anytime if four weeks have elapsed
since the first dose and both doses are administered beginning at or after
age 12. Those children who have not previously received the second dose
should complete the schedule by age 11-12 years.
Varicella (chicken pox). Immunize all children age 1 year and older, including
adolescents who have not had chickenpox. Immunization is not required
in people with a history of natural disease (chickenpox).
Susceptible children age 13 and older should receive two doses at least
4-8 weeks apart.
• The sponsor’s current military orders. Orders dated 36 months
prior to the beginning of next school year will require an amendment,
updated orders or the registrar can provide a form to be completed by
the sponsor’s unit.
• Documentation of the sponsor's housing assignment in permanent
quarters, either by a housing letter, Housing Assignment Voucher, or the
completed front page of the RCI lease agreement or a 90/180-day letter
from the appropriate Housing office. The document must list the child
as a dependent with authorization to live in housing. No child will be
permitted to start school without this documentation.
• Records from previous school to include last report card/progress
report. These records are needed, but, will not stop the progress of your
child from entering school.
